Longevity care is almost entirely self-pay. Here is the actual spread.

These ranges come from the 30 providers in the registry that publish or disclose pricing. We list price bands as the provider states them; we do not negotiate, resell or take commission on any programme.

Five price tiers

What the money buys at each level

Direct diagnostics

$350 – $2,500

A single test bought without a programme: an epigenetic clock, a DEXA scan, a whole-body MRI or a cancer-signal blood test. You get a result and a report; interpretation and follow-up are on you.

Ask who reads the result with you, and what a positive finding triggers.

One-time assessment

$5,000 – $15,000

One to three days of testing — bloods, imaging, VO2max, body composition — closed by a physician consultation and a written plan. No ongoing care included.

Ask which tests are actually included, and what the plan costs to execute.

Annual programme

$15,000 – $40,000 per year

Membership with a baseline assessment, quarterly follow-up, prescribing and coaching. This is the dominant model among dedicated longevity clinics.

Ask about the minimum commitment, what repeats each year, and cancellation terms.

Hospital executive health

From $1,200

A structured day inside an academic medical centre, with credentialed physicians and an internal referral pathway. Narrower in scope than a longevity programme, and the cheapest serious entry point.

Ask which of the eleven procedures are included, and which are not offered at all.

Premium concierge

Upper range, on request

Unlimited access, a dedicated physician, at-home logistics and travel coordination. Pricing is quoted individually and rarely published.

Ask for the quote in writing, itemised, before any consultation fee.

The gap

Four costs that sit outside the headline price

  • Prescriptions and compounded therapies

    Hormones, peptides and infusions are usually billed on top of the membership, monthly.

  • Repeat testing

    The second and third round of bloods, scans or clocks is where the annual cost quietly doubles.

  • Supplements

    Often sold in-house. Ask whether the clinic earns margin on what it prescribes.

  • Downstream care

    A finding on a scan leads to specialist visits, biopsies and imaging that no programme fee covers.
